REGISTRATION FORMS AND DATES

A registration form can be downloaded and printed from your computer. The forms below are in Adobe Acrobat format (.pdf). (You need Acrobat Reader installed on your computer to properly display and print a form. The program can be downloaded for free from the Adobe website.) Each form will open in a new (separate) window.

UNDERGRADUATE REGISTRATION FORMS:

Registration for FALL 2008 began on Monday, April 28, 2008. Register by Friday, June 20 to avoid a late fee. See the Academic Calendar on this website for deadlines and other important dates, and check this calendar and the portal periodically for any changes and updates.

The fall 2008 course schedule is available on this website and through the web portal. No registration forms for fall will be accepted before Monday morning, April 28.

MFA students can obtain registration forms from the Office of the Dean.

IMMUNIZATION

PLACEMENT

To fulfill the mathematics or English requirement in an undergraduate program at NYSID, students must take the placement test before registering for mathematics or English, unless you have received approval from NYSID to transfer an equivalent course from a previous college. Check the placement test schedule on the Academic Calendar on this website or contact the placement advisor for additional information. Students must register for the test in the Office of the Dean before the test date. Students may not take the mathematics or English placement test more than once.
